  1. The breast is made up of glands called lobules that can make milk and thin tubes called ducts that carry the milk from the lobules to the nipple. Breast tissue also contains fat and connective tissue, lymph nodes, and blood vessels.

The most common type of breast cancer is ductal carcinoma, which begins in the cells of the ducts. Breast cancer can also begin in the cells of the lobules and in other tissues in the breast. Invasive breast cancer is breast cancer that has spread from where it began in the ducts or lobules to surrounding tissue.

In the U.S., breast cancer is the second most common cancer in women after skin cancer. It can occur in both men and women, but it is very rare in men. Each year there are about 2,300 new cases of breast cancer in men and about 230,000 new cases in women.

  2. Breast Cancer constitutes a major public health issue globally with over 1 million new cases diagnosed annually, resulting in over 400,000 annual deaths and about 4.4 million women living with the disease. It also affects one in eight women during their lives. It is the commonest site specific malignancy affecting women and the most common cause of cancer mortality in women worldwide. It is also found in men but not very common. Statistics available in Nigeria are largely unreliable because of many factors that have not allowed adequate data collection and documentation; but according to numbers provided by Globocan in 2002, Breast Cancer is responsible for about 16% of all cancer related deaths in Nigeria. In a recent publication by Okobia et al (2006):”Late presentation of patients at advanced stages when little or no benefit can be derived from any form of therapy is the hallmark of breast cancer in Nigerian women.” This is indeed a worrisome trend and it appears to be the norm in Nigeria.

  4. Hormone therapy for breast cancer

Hormone therapy is another form of systemic therapy. It is most often used as an adjuvant therapy to help reduce the risk of the cancer coming back after surgery, but it can be used as neoadjuvant treatment, as well. It is also used to treat cancer that has come back after treatment or has spread.

A woman's ovaries are the main source of the hormone estrogen until menopause. After menopause, smaller amounts are still made in the body's fat tissue, where a hormone made by the adrenal gland is converted into estrogen.

Estrogen promotes the growth of cancers that are hormone receptor-positive. About 2 out of 3 of breast cancers are hormone receptor-positive — they contain receptors for the hormones estrogen (ER-positive cancers) and/or progesterone (PR-positive cancers). Most types of hormone therapy for breast cancer either lower estrogen levels or stop estrogen from acting on breast cancer cells. This kind of treatment is helpful for hormone receptor-positive breast cancers, but it does not help patients whose tumors are hormone receptor negative (both ER- and PR-negative).

  5. Cancer may develop when the immune system breaks down or is not functioning adequately. Immunotherapy (also called biological therapy and biotherapy) uses the body's immune system to fight cancer. Immunotherapy works by either stimulating your immune system to attack cancer cells or providing your immune system with what it needs, such as antibodies, to fight cancer.

    Common types of immunotherapy include:

Monoclonal antibodies: These are man-made versions of immune system proteins. Antibodies can be useful in treating cancer because they can be designed to attack a very specific part of a cancer cell.
Cancer vaccines: Vaccines are substances designed to trigger an immune response in the body against certain diseases.
Non-specific immunotherapies: These treatments stimulate the immune system in a general way to increase activity against cancer cells. Some examples include man-made versions of cytokines, a chemical in immune cells, such as interleukins and interferons.

New knowledge about how the immune system works is helping guide how we use immunotherapy to treat cancer.

  6. Lung Cancer
    Lung Cancer

There are three main types of lung cancer. Knowing which type you have is important because it affects your treatment options and your outlook (prognosis). If you aren’t sure which type of lung cancer you have, ask your doctor so you can get the right information.
    Non-Small Cell Lung Cancer

This is the most common type of lung cancer. About 85% of lung cancers are non-small cell lung cancers. Squamous cell carcinoma, adenocarcinoma, and large cell carcinoma are all subtypes of non-small cell lung cancer.
    Small Cell Lung Cancer

Small cell lung cancer is also called oat cell cancer. About 10%-15% of lung cancers are small cell lung cancers. This type of lung cancer tends to spread quickly.
    Lung Carcinoid Tumor

Fewer than 5% of lung cancers are lung carcinoid tumors. They are also sometimes called lung neuroendocrine tumors. Most of these tumors grow slowly and rarely spread.

  7. Extra fat in the body can have harmful effects, like producing hormones and growth factors that affect the way our cells work. This can raise the risk of several diseases, including cancer. It's thought that more than 1 in 20 cancers in the UK are linked to being overweight or obese.
Research has shown that many types of cancer are more common in people who are overweight or obese, including:

breast cancer, in women after the menopause
    bowel cancer
    womb cancer
    oesophageal (food pipe) cancer
gastric cardia cancer (a type of stomach cancer)
    pancreatic cancer
    kidney cancer
    liver
probably - gallbladder, ovarian and aggressive prostate cancers

This list includes 2 of the most common types of cancer – breast and bowel cancers - and 3 of the hardest to treat – pancreatic, oesophageal and gallbladder cancers.

  8. A common misconception that most people have about cancer is that suffering from cancer is the end of life. However it is not true as with numerous advances in the diagnosis and treatment options, you can improve the quality of life and get treated (if the condition is diagnosed in early stage). With advances in chemotherapy, radiotherapy, surgery, hormonal therapies and vaccines, the current treatment options for cancer have better mechanism of action and fewer side effects. Here are some of the significant advances and research in the field of cancer treatment.
    Combination Chemotherapy

Currently, combination chemotherapy is a standard treatment used for various types of cancer. It is known to be one of the key reasons for increasing survival and remission rates in cancer treatment. Here’s what to expect at your first chemotherapy session.
For example, the introduction of combination chemotherapy containing cisplatin has led to the remission in the number of people with testicular cancer to approximately 95%.
This revolutionary drug combination therapy is so effective that it has raised the long term survival in metastatic testicular cancer patients to 80%, as compared to thirty years ago where 95% patients died within one year of diagnosis.
Apart from testicular tumours, Cisplatin has improved the outlook in other cancers especially ovarian cancers, oral cancers, lung cancers, etc.

    Cancer treatment vaccines

Recently, the FDA approved the first ‘cancer treatment vaccine’ for advanced prostate cancer. There are a number of cancer treatment vaccines that are being tested that may have the potential to treat non-small cell lung cancer, pancreatic cancer, ovarian cancer, melanoma and multiple myeloma.

  9. Cancer Prevention Vaccines

Vaccines targeted against HPV (Human Papilloma Virus), which is responsible for 99.9% of cervical cancers, are now available in the market. There are many strains of HPV virus, which cause the cancer. These vaccines are effective against strains no. 16 and 18, which are responsible for around 85% of cervical cancer cases in India. However one must note that, vaccines are not a replacement for regular PAP smears, which must go on as per guidelines. Read more about can vaccination really prevent cervical cancer?
    Molecular target agents

The normal body cells divide unnaturally, when a cancer forms and certain ‘factors’ or ‘cellular agents’ or even changes in normal ‘genes’ stimulate this growth of cancer. Recent advances in technology have enabled us to identify such ‘agents’ in different cancers. This has lead to the development of drugs targeted against such ‘agents’ to block them and restrict the unnatural multiplication of these cells. This is called as ‘Targeted Therapy’. To note, this is a major advance in biotechnology, because these so called ‘targets’ are really very minute (tiny sockets located on a cell, so you can imagine how small they must be!), and developing drugs against them needs very sophisticated techniques. Hence, these drugs are still very costly. Here’s what you should know about beating cancer with naturopathy and herbal medicine.

The first such targeted agent in clinical use was Trastuzumab, used in those breast cancers that over-express HER2 protein. HER2 positive breast cancers tend to be very aggressive; addition of Trastuzumab (Herceptin) has improved survival in such cancers. Currently, there are more than 50 such targeted agents available for various cancers. Some of the common target drugs include –

Imatinib mesylate for chronic myelogenous leukemia and gastrointestinal stromal cell tumors (GIST)
Sunitinib for advanced kidney cancer and imatinib-resistant GIST
Bevacizumab for advanced colorectal, non-small cell lung and kidney cancers
Bortezomib for multiple myeloma and a type of non-Hodgkin lymphoma

  10. #2 ADVANCES IN RADIATION THERAPY

There have been a number of advancements in the field of radiation therapy – 3D conformal radiation therapy, stereotactic radiosurgery, and brachytherapy (radioactive seeds), which are designed to deliver high doses of radiation to tumors while minimizing the doses delivered to nearby healthy tissue. These new techniques have helped preserve greater amount of tissues, organs and limbs. Treatments to reduce the side effects of radiotherapy such as pain, mouth sores, nausea, and vomiting, have also been devised. The latest advances in radiation therapy that grabbed eyeballs are –

Cytotron: It is a revolutionary new treatment that promises to relieve cancer patients of their pain without the side effects of conventional treatments. Developed by Dr. R.V.Kumar of Ludhiana, the device arrests cancer cell growth by using a technique called degenerative tissue engineering. Instead of the very high frequency ionising radiation used in radiotherapy, RFQMR uses radio or sub-radio frequency, low power, non-ionising, non thermal electromagnetic waves. Read more about diet for cancer patients during and after treatment.

Stereotactic body radiation treatment (SBRT): Also known as stereotactic ablative radiation therapy, SBRT appears to be a very useful new approach for many otherwise difficult to treat cancers such as in the lung. This technique uses various angles to treat the tumor. In this technique the tumor is imaged and the robotic arm is adjusted to directly attack the cancer and not the normal tissue.

Robotic radiosurgery: An advancement of the CyberKnife, robotic radiosurgery is used to treat benign tumors, malignant tumors and other medical conditions. Invented by John R. Adler, it is a system that delivers radiotherapy with a lot more accuracy as compared to the traditional therapies. CyberKnife works in a two step fashion – it firstly produces highly accelerated particles which are then focussed onto the cancereous area through a robotic arm.

  11. #3 ADVANCES IN HORMONAL THERAPY

The estrogen receptor-positive breast cancer cells can be stimulated to grow by the hormone estrogen. However, SERMs (Selective Estrogen Receptor Modulators) interfere with this type of stimulated growth by preventing estrogen hormone from binding to its receptor. Hormonal therapy with tamoxifen and Aromatase Inhibitors (Letrozole, Anastrozole) is now standard in the treatment of women with estrogen receptor-positive breast cancer, both as adjuvant therapy and in the treatment of advanced disease. These drugs have significantly improved survival rate in breast cancer. Read in detail about 5 interesting facts about biological therapy for cancer.

    #4 ADVANCES IN CANCER SURGERIES

    Breast Conservation surgery (BCS)

Currently, about 70% of breast cancers will be feasible for BCS (followed by compulsory radiation therapy). Even when a tumour is large, chemotherapy can be given prior to surgery, which achieves a good reduction in size of the tumour and makes it feasible for BCS.

  12. The excessive pressure on your artery walls caused by high blood pressure can damage your blood vessels, as well as organs in your body. The higher your blood pressure and the longer it goes uncontrolled, the greater the damage.

Uncontrolled high blood pressure can lead to:

Heart attack or stroke. High blood pressure can cause hardening and thickening of the arteries (atherosclerosis), which can lead to a heart attack, stroke or other complications.
Aneurysm. Increased blood pressure can cause your blood vessels to weaken and bulge, forming an aneurysm. If an aneurysm ruptures, it can be life-threatening.
Heart failure. To pump blood against the higher pressure in your vessels, your heart muscle thickens. Eventually, the thickened muscle may have a hard time pumping enough blood to meet your body's needs, which can lead to heart failure.
Weakened and narrowed blood vessels in your kidneys. This can prevent these organs from functioning normally.
Thickened, narrowed or torn blood vessels in the eyes. This can result in vision loss.

  14. If there's one thing that former president Jimmy Carter's successful treatment for cancer proved, it is that scientists have made a marked improvement in treating this deadly disease.

Experts told USA TODAY that the drug used in his treatment called pembrolizumab, also known as Keytruda, is part of a new class of drugs that fight cancer by boosting the body's immune system. Timothy Turnham, executive director of the Melanoma Research Foundation, revealed that it is one of the eight new melanoma drugs approved since 2011.

Even though none of the new drugs had been proven to cure cancer, patients and cancer advocates have celebrated their arrival. It is noteworthy that the FDA did not approve these drugs at all from 1998 to 2011.

Researched in Israel by Professor Jacob Schachter of the Ella Institute for melanoma treatment and research at the Sheba Medical Center in Tel Hashomer, Keytruda is part of a promising new class of drugs called immunotherapies, which harness the body’s immune system to help fight cancer.

The US-based Merck pharmaceutical company’s injectable biotech drug works by blocking a protein found in certain tumors called PD-1, which inhibits the body’s natural response to cancer cells.

  16. Although it has shown a lot of promise, the new immune therapy drug used to treat former President Jimmy Carter’s cancer is still a ways off from reaching the status of a “miracle drug.”

Several medical experts told Healthline it’s too early to tell how much credit the drug Keytruda should get for Carter’s successful treatment. The medication is one of a new class of cancer-fighting drugs that are designed to sic the body’s own immune system on cancers. In Carter’s case, the drug appears to have shrunk the tumors in his brain to be undetectable. Now Carter and his doctors will wait to see if the melanoma returns.

Not everyone who might benefit from Keytruda will have access to it. Some insurance providers do not cover the medication, which is estimated to cost $150,000 a year.

  23. World Cancer Day is an international day marked on February 4 to raise awareness of cancer and to encourage its prevention, detection, and treatment. World Cancer Day was founded by the Union for International Cancer Control (UICC) to support the goals of the World Cancer Declaration, written in 2008. The primary goal of the World Cancer Day is to significantly reduce illness and death caused by cancer by 2020.[1]

World Cancer Day targets misinformation,[2] raises awareness, and reduces stigma.[3] Multiple initiatives run on World Cancer Day to show support for those affected by cancer. One of these movements is #NoHairSelfie, a global movement to have "hairticipants" shave their heads either physically or virtually to show a symbol of courage for those undergoing cancer treatment.[4] Images of participants are then shared over social media. Local events also take place

Researchers discovered that even though cancer cells mutate wildly within a person's body, the cancer cells within each patient also have common mutations -- ones that could be isolated and fought off by certain immune cells.

Think of it this way: A patient's cancer cells all start off with the same tree trunk, but then grow different kinds of branches. The new research shows certain immune cells can "chop the tree at the trunk rather than just pruning the branches," Dr. Sergio Quezada told CNN.
